There are countless ways artificial intelligence is now being applied to the practice of therapy. Clinicians are regularly presented with new tools to optimize efficiency with paperwork, note taking, client assessment, and communications. Increasingly, clients are coming to the therapy office having already sought input from AI tools like Chat GPT, Copilot, Gemini, or Grok. 

How AI is Being Used by Therapists

Clinicians and clients alike need to consider the impact AI has on the therapeutic process. For example, some clinicians use tools that record sessions with clients. These recordings create transcripts and AI-generated summaries to assist clinicians in their assessment of session content. Recordings are typically deleted after appropriate documentation is generated, but clients should be notified and give their consent before such tools are put into practice.

Protecting Privacy and the Therapeutic Relationship

Aside from legitimate concerns about privacy, AI tools in the therapy room also impact the therapeutic relationship. Confidentiality is a foundational hallmark of therapy for good reason—we cannot take it lightly to surrender that trust for the expediency of a lighter workload. Clinicians bear the responsibility to protect the best-interests of their clients, even if it requires greater administrative effort.* 

How AI May Affect Therapists

It’s also important to consider how AI tools impact the clinician. Some argue these tools save valuable time, allowing clinicians to focus on clients rather than administrative tasks. There’s no question that clinicians clock considerable non-clinical hours attending to administrative work related to client care. 

But efficiency is not always the most important goal. For a clinician, active listening, summarizing, and assessing are critical skills that require consistent and frequent use to remain sharp. The therapist grows along with the client as insights facilitate processing and the growth of wisdom over time. The temptation is strong for a time-pressed clinician to use an AI-generated assessment rather than endure the more taxing process of thinking a case through. AI tools do provide useful help, but it would be an error to assume they have no cost. Clinicians will need to establish habits to hone their skills and avoid atrophy.

How AI May Affect Clients

Finally, AI is having a significant impact on clients as well. Like clinicians, clients may rely on AI in lieu of doing the harder—and arguably more important—work. Clients have shared about using AI to approach difficult conversations with a loved one, glean insights about their spouse’s habits, and generate scripts to help them navigate communication gaps

While sometimes AI has been helpful with these tasks, I’ve also seen AI cause clients to become more entrenched in their own assumptions and less curious about their spouse’s input. Why bother with the tense conversation for an answer that AI can provide with ease? The focus tends to lean toward assessment and evaluation rather than intimacy, connection, and mutual understanding.  

Technology Should Support—Not Replace—Human Relationships

Every new tool is an invitation to evaluate rewards and costs. Our human fascination with developing technologies can lead us to adopt quickly and ask questions later. But when we invite technology into the most intimate personal spaces—the bedroom, the kitchen table, the therapy office—we need to be wise in preserving that most precious and irreplaceable of treasures: human relationships.

Can AI replace a licensed therapist?

No. AI can provide general information, journaling prompts, or communication suggestions, but it cannot replace the clinical judgment, empathy, ethical responsibility, and therapeutic relationship provided by a licensed mental health professional. Therapy is built on trust, human connection, and individualized care.

Is it okay to use AI for mental health advice?

AI can sometimes help people organize their thoughts or learn about mental health topics, but it should not be considered a substitute for professional therapy or crisis support. If you're experiencing ongoing anxiety, depression, relationship difficulties, trauma, or other mental health concerns, working with a licensed therapist is the safest and most effective approach.

Do therapists use AI during counseling sessions?

Some therapy practices use AI-assisted documentation tools with client knowledge and consent. At Firm Foundation Family Services, we currently do not use AI tools during therapy sessions or for clinical documentation because we prioritize protecting the therapeutic relationship and client confidentiality.

Is therapy confidential?

Yes. Confidentiality is one of the foundations of therapy. Licensed therapists follow ethical and legal standards designed to protect your privacy, with limited exceptions required by law. If you have questions about confidentiality, your therapist can explain how your information is protected before treatment begins.

Where can I find a therapist in Ashburn or Loudoun County?

Firm Foundation Family Services provides in-person counseling at our Ashburn office for individuals, couples, families, and children throughout Ashburn, Leesburg, Sterling, Brambleton, Aldie, Purcellville, and surrounding Loudoun County communities. We also offer virtual therapy for clients in Virginia, and our parent coaching services are available worldwide.

Does Firm Foundation Family Services offer in-person and virtual therapy?

Yes. We offer in-person counseling at our Ashburn, Virginia office for clients throughout Loudoun County and surrounding Northern Virginia communities. Virtual therapy is also available for eligible clients throughout Virginia, and some clinicians are licensed to serve clients in additional states.
